Hemorrhoids are enlarged veins in the rectum and anal area. There are two types of hemorrhoids: External and Internal.  Internal hemorrhoids are generally not painful unless they protrude from the anal canal.  Most patients will complain of painless bleeding after a bowel movement and may feel like something is protruding from the anus.  External hemorrhoids are located near the anus and often can become painful and enlarged if a blood clot develops.  When that occurs, it is called a “thrombosed hemorrhoid.”

We are all born with these veins in and about the anus and rectum. Constipation, prolonged time on the toilet, straining with bowel movements, pregnancy, diarrhea, excessive laxative use or enemas can all trigger symptomatic hemorrhoids. Increased pressure causes bulging of the veins, leading to weakness of the vessel wall.

Most people with internal hemorrhoids complain of itching, mucous, painless bleeding and something protruding from the rectum. Symptoms are most pronounced after a bowel movement. In the case of a thrombosed external hemorrhoid, you may experience severe pain along with a swollen and tender lump in the external anal area.

If you develop symptoms from an internal hemorrhoid you will undergo an examination in the office of Premier Surgical Associates in Palm Springs, La Quinta & Hemet CA. This may include inspection of the external anal area and anoscopy (small lighted scope inserted into the rectum) to visualize internal hemorrhoids. In most cases, hemorrhoids can be managed without surgery. This includes dietary and behavior modifications such as increasing fiber and water intake and avoiding long periods of time sitting on the toilet. Stool softeners will help decrease straining and suppositories and sitz baths can be used to decrease swelling. If symptoms do not improve or if hemorrhoids are very large, surgery can be offered. Surgical procedures include rubber band ligation, Transanal hemorrhoidal dearterialization and excisional hemorrhoidectomy. These options can be discussed in detail with your surgeon and he or she will decide which option is best suited for you.

Thrombosed external hemorrhoids are also treated with warm sitz baths, stool softeners, and fiber. It takes 1-2 weeks for these lumps to go away and they can remain painful for several days. If the hemorrhoid is severe, it can be removed with the clot in the operating room.

There are many disease processes that can mimic hemorrhoid symptoms, so it is best to be evaluated by a physician in one of the offices of Premier Surgical Associates in Palm Springs, La Quinta & Hemet CA to make the correct diagnosis.
